The Timbers of Shorewood Transitional Care Program has been developed by an interdisciplinary team of experts to offer a safe, temporary home while the guest continues to recover and restore his or her health.
The Timbers of Shorewood staff members partner with healthcare providers and insurance companies to ensure coordinated care and professional assistance for residents. Professional staff members work with the physician, physical therapists and nurses for a worry-free transition from the hospital, to The Timbers, and then home.
Transitional care amenities include:
- Physical therapy
- Occupational therapy
- Speech therapy
- Skilled nursing
- Medication reconciliation
- Telemonitoring for management of chronic disease
- Nursing assistance
- Scheduling and transportation for follow up appointments
Other services available include personal assistance, an emergency call system, assistance in bathing and grooming, and personal laundry. The Timbers of Shorewood has physicians on-site and 24 hour-a-day staffing and security. Each apartment has a private telephone and housekeeping services. The Timbers also offers a fitness center, wellness programs, chef-prepared meals, an on-site beauty salon and barber shop, and daily social programs.
